As Department Chaplain and Graves Registration Chairman it is my duty to be certain all departed Brothers and Sisters are honored with a memorial card to their family. We need your help to be certain our deceased members are reported and recorded appropriately.  This information is very important to send condolences to the grieving family and so our records are accurate.


At the Auxiliary and District levels, it’s vital this information is reported promptly and completely. Customarily, the job of Graves Registration Chairman is the responsibility of the Chaplain of the Auxiliary.

The simplest way to report is online at Utilize the following steps for online reporting:

  1. Online Reporting
  2. Graves Registration
  3. Fill in the Form

You will need your member’s full name and membership number, life or annual member along with the date of death.  Please be certain your District Graves Registration Chairman receives a copy of the report.

Treasurers Report on MALTA, our National Website. Remember, reporting the death of a member through MALTA, DOES NOT notify our Department.

If you do not report online, please use the form in the program book.  Destroy previous forms as the Chairman changes yearly.  Please use the correct form for 2020-2021 year; complete all parts of the form and mail to the appropriate person.
