This is your Graves Registration Form.

NOTE: When reporting deceased members, the "VFW AUXILIARY VFW CHANGE REQUEST FORM" must be completed. To report online, fill out the "Graves Registration Form" under the "Online Reporting" tab (and this is the easiest and most efficient method). If you are not reporting online, print the Change Request Form from the Department website (or your Program Book), and mail it. On the website, this form is found under "Reports and Forms; Treasurer's Information; Change Request Form"). Mail to Linda Lorenz, Department Treasurer 140 W. Ann Street, Somonauk, IL 60552. If the form is not completed, deceased members will not be removed from the Department and National rosters, which may affect your Auxiliary totals for the next year. Note that this form changes every year! Always use the current year's form. **Membership Number must be included on this form.
